Care Transitions Manager

The Care Transitions Manager is responsible for leading and guiding the Collaborative Care Model for rehab patients from the Masonic Home, community and hospital through their discharge process from rehab including follow up at home and across our Continuing Care Community. Education and experience for this position is a Licensed Nurse with a minimum of 2 years of patient experience in a long term care setting. Assistant Director of Nursing

The Assistant Director of Nursing is responsible, as delegated, for the overall nursing care delivery system including quality of care, fiscal accountability, staff performance in accordance with nursing standards of practice, federal, state and local regulations as well as, Masonic Pathways policies and procedures. Graduation from an accredited school of nursing and current, valid Registered Nurse license by the State of Michigan is required. A successful candidate can provide evidence of strengths in leadership, communication, decision making skills and conflict resolution along with having a minimum of two years of experience in a leadership role. 

Live Better At Home Call Center Specialist

Live Better at Home is accepting applications for the position of Live Better At Home Call Center Specialist. The goal of Live Better at Home™, a product of the Michigan Masonic Home dba Masonic Pathways, is to bring people and services together in an effort to help them meet their needs and achieve their personal goals. Live Better at Home empowers people with the ability to make informed choices by providing assistance, expert advice, referral, support, advocacy, and navigation to access resources and community services that facilitate independence, self-reliance and dignity. The program’s goal is to bridge the gap between community resources and caller needs to promote health, physical, social, financial, and personal enrichment to enhance their quality of life. 

The Live Better At Home Call Center Specialist will work as an integral member of the Live better at Home Information and Referral Call Center at Masonic Pathways to provide a single point of contact, that is capable of researching and finding solutions customized for each individual caller and referral. They will provide general information and referral via the toll free phone line, email or web-chat and will make out-bound call-back, follow-up and advocacy calls.  

A Bachelor's of Social Work or Bachelor's degree (B. A. or B. S.) from four-year college or university is required; and one to three years related experience and/or training; or equivalent combination of education and experience.  

Unit Manager

Masonic Pathways is accepting applications for a Unit Manager. The Unit Manager is accountable to the Director of Nursing for the day to day activities and tasks performed by self and assigned staff. They are also accountable for overall resident care and staff supervision through the appropriate and consistent follow through of resident care plans for the unit as assigned.  Unit Manager is accountable for achieving outcome results as outlined for the unit as assigned through the CQI process.  They will perform related duties as required to meet the Mission, Vision and Values of Masonic Pathways. Required training and experience to include graduation from an accredited school of nursing and must possess and maintain a current, valid registered nurse license required by the State of Michigan. Candidate must possess excellent organizational, communication and leadership skills and have at least 2 years of supervisory experience in either a long term care or acute care setting. 

RN & LPN Unit Nurses

Masonic Pathways is seeking RN’s & LPN’s for afternoon and 12 hour PM shifts. Positions are available in our Long Term Care, and Memory Care. We offer competitive wages and excellent benefits. Unit nurse responsibilities include but are not limited to medical record documentation, provide care according to individual resident care plans, and monitoring CNA personnel on the assigned unit. six months to one year related experience or training; or equivalent combination of education and experience.  Experience in long term care preferred.

We are currently seeking Contingent Respiratory Therapists for our expanding Assisted Breathing Unit. Job duties include but are not limited to communications with nurses and physicians about changes in patient condition, ability to perform respiratory therapy in all patient care areas, provide care and perform education on ventilator management, tracheostomy care, suctioning, and ventilator weaning. Must possess a current and valid Respiratory Therapist license issued by the State of Michigan is required


Contingent External Transporter

We are seeking a Contingent External Transporter for our Transportation department to provide transportation for our residents outside of Masonic Pathways. They will work on an as needed basis for various shifts. Specific duties include but are not limited to transporting residents to and from daily external appointments and activity trips at various locations by bus and maintaining the interior of the vehicle. A current CDL group C with Passenger endorsement and/or Chauffeur license is required.
