Associate Director of Preventative Health

If you are a responsible and self-starting individual, then this opportunity is perfect for you. Masonic Pathways has an Associate Director of Preventative Health position available for qualified and eager applicants. This position works in collaboration with the Unit Manager ensuring appropriate assessment for resident needs and services. As the Associate Director of Preventative Health, the position is responsible for the facilitation and coordination of preventative health measures including: flu vaccine program, infection control program, wound program, and LIFT program.


Essential Duties include but are not limited to:

  • Identifying, tracking, monitoring and investigating infection, accidents and incidents to determine probable causes and recommends actions.
  • Checking all units for new or recurrent infections. Collects data from units and tracks data in an organized fashion to identify patterns and trends.  
  • Leading the implementation of the “Prevention” focused initiatives (falls, skin, infection, and pain).  
  • Analyzing Facility and Community Acquired Wound data for patterns/trends, making recommendations and submitting written report to Administration. 

Education and/or Experiences include:

  • Required training and experience to include graduation from an accredited school of nursing and must possess and maintain a current, valid Registered Nurse license required by the State of Michigan.
  • Ability to demonstrate critical thinking skills.
  • Minimum two years’ experience in a leadership role. Can provide evidence of strengths in leadership, communication, decision making skills and conflict resolution.
  • Can provide demonstration of job performance as related to higher level management skills to include multiple tasking, time and fiscal management and professional development strengths.

PACE – Executive Director (Central Michigan)

PACE – Central Michigan is seeking an Executive Director for a startup collaboration between Michigan Masonic Home and Presbyterian Villages of Michigan with potential participants beginning early fall of 2018. The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible. PACE serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE enrollee needs nursing home care, the PACE program pays for it and continues to coordinate the enrollee's care. We encourage fellowship and independence while serving seniors and their families with dignity and respect.

The Executive Director will be responsible for developing and implanting the infrastructure and proper management tools to effectively run the PACE program. This position will align the overall operations of PACE – Central Michigan in accordance with current federal, state and local standards, guidelines, and regulations that govern such programs and senior services and to assure the highest quality of care and positive relations among the fraternity, community, employees, patients and families.
The successful candidate will have, at a minimum, the following qualifications:

  • Bachelor’s Degree in business administration, or a health or human services field, or Registered Nursing Degree/diploma, professional license and Master’s degree preferred.
  • Five (5) years of health care experience, and three (3) years of management experience, preferably in a geriatric care setting.  
  • Working knowledge of the PACE Regulation and all other regulatory requirements.  
  • At least one (1) year experience working with the frail or elderly population. 
  • Related experience in the direction of a health care program.
  • Working knowledge of health care delivery and financing systems, including Medicaid, Medicare, waiver programs, prospective payment systems, and health management organizations.

Certified Medical Assistant - Contingent

Masonic Pathways is in search of a caring and hard-working individual with a current State of Michigan LPN or Certified MA License to be a part of our team as a Wellness Coordinator. The Wellness Coordinator is responsible for coordinating and providing wellness care for residents residing in Residential Living while building supportive relationships with residents, family members, and service providers.

Some essential responsibilities of this position include, but are not limited to:

  • Visiting each resident immediately following an acute hospital stay or a Healthcare Center stay initiating adjustment of Wellness Service Plan.
  • Supporting resident’s desire to age in place 
  • Developing, managing and auditing individualized Wellness Service Plans for residents upon admission and updated as necessitated and regulated by the State of Michigan.\Coordinate referrals for home care, hospice and in-home technology services. 
  • Coordinating referrals to the Medicare Part B program. Monitor the initiation and compliance with physician ordered therapies. 
  • Promoting optimizing function of all residents through physical activity and wellness activities

Education/Experience would be certification through the Certifying Board of the American Association of Medical Assistants (AAMA), or valid LPN registration by the State of Michigan. Experience in a licensed Home for the Aged setting, Knowledgeable of Home for the Aged rules and regulations.

Unit Assistant (Full & Part Time)

Masonic Pathways is in search of a motivated team player that can assist our residents with great care. A Unit Assistant assists a Certified Nursing Assistant in the day to day functions of the unit. This can include non-direct resident care as well as resident assistance.

Job Responsibilities consist of:

  • Daily housekeeping to maintain a safe and clean environment throughout the assigned area 
  • Assist in emptying linen carts, stocking gloves and dining rooms, and cleaning utility rooms 
  • Assist residents with passing water, passing supplements, changing oxygen tubing, transporting residents to dining room and unit activities 
  • Responds to call lights and relays all resident direct care requests to Certified Nursing Assistant or staff nurse 
  • Accompanies independent residents on walks within the facility

A Unit Assistant must be able to work independently and have a customer service attitude. There is no prior experience or education needed for this position. A high school diploma or GED preferred. Interested applicants may apply online by October 30, 2017.


Associate Director of Clinical Assessments

Masonic Pathways is seeking an Associate Director of Clinical Assessments. This individual will ensure there is a proactive approach to resident care. The position needs a strong leader with great attention to detail. Overseeing resident care, including accuracy of records with actual resident status, is an important part of this position. The Associate Director of Clinical Assessments also notifies Nurse Managers of lapses and omissions, responding as directed, and assists in auditing documentation.
Key Responsibilities include, but are not limited to:

  • Oversees completion of nursing assessments on all new, readmission’s and updates when significant changes occur and responds to MDS items per established P&P. 
  • Ensures RNAC staff make referrals to the physician for Rehab and Restorative programs as indicated after assessment of residents’ status. 
  • Reviews and audits care plans for timeliness, accuracy, quality and content of nursing notes. 
  • Audits physician certifications for accuracy and completeness. 
  • Audits to ensure RNAC’s review immediate needs care plans are identified and accurate on admission. 
  • Maintains timely MDS assessments by working in collaboration with therapy and reviews need for grace days. 
  • Educates the IDT on changes related to the MDS. 
  • Manages weekly Medicare meeting with IDT. 
  • Manages LOC tools for Medicaid admissions. 
  • Audits to ensure care plan emanating from the completed MDS and CAA’s is developed in a functional model and is strongly holistic. 
  • Attends in-services at a minimum of 12 contact hours annually
The Education/Experience Qualifications include:
  • Greater than 5 years’ experience in MDS coordination. 
  • Associate’s degree (A.A.) or equivalent from two-year college; and five years related experience and/or training as a MDS coordinator; or equivalent combination of education and experience. MDS certification preferred.

Contingent Respiratory Therapist (12 hour shifts)

We are currently seeking a contingent Respiratory Therapist for our expanding Assisted Breathing Unit. Job duties include but are not limited to communications with nurses and physicians about changes in patient condition, ability to perform respiratory therapy in all patient care areas, provide care and perform education on ventilator management, tracheostomy care, suctioning, and ventilator weaning. Must possess a current and valid Respiratory Therapist license issued by the State of Michigan is required

 

Contingent External Transporter

We are seeking a Contingent External Transporter for our Transportation department to provide transportation for our residents outside of Masonic Pathways. They will work on an as needed basis for various shifts. Specific duties include but are not limited to transporting residents to and from daily external appointments and activity trips at various locations by bus and maintaining the interior of the vehicle. A current CDL group C with Passenger endorsement and/or Chauffeur license is required.
